Moving
Services
Services Provided
Inter- and intra-office moves of furniture and equipment for all
OUHSC departments. Non-OUHSC department moves must first receive
approval, and the department will be charged for any services provided.
Removal and disposal of operative or inoperative equipment and furniture
will be accomplished by methods governed by State and University
policies.

8:00 AM to 4:30 PM Monday through Friday
Excludes University recognized holidays

Guidelines
A Request for Moving Services form (OU-5093) must be completed and
authorized by the requesting department head. This form must be
completed with the appropriate inventory and serial numbers documented,
and submitted to Equipment Inventory, SCB224N.
All requests must be submitted a minimum of one week prior
to the proposed date of required services.
To dispose of equipment or furniture, the department may also need
to complete a Removal of
Equipment form in addition to The Request for Moving Services
form. The Removal of Equipment form must be completed
in order to remove items from the University inventory records.
The Request for Moving Services form (OU-5093) and Removal of Equipment
form should be submitted to Equipment Inventory, SCB224N.
